Enrolments Process

Kindergarten 2022

Kindergarten is the beginning of a child’s journey through school where they learn important early numeracy, literacy and social skills. Children can start Kindergarten when turning four years old by 30 June of the year they attend. 

KINDY ENROLMENT APPLICATION FORM

Parents are asked to complete the below application and the required supporting documentation before Friday 23rd July 2021, to be considered for placement into Kindergarten for 2022. Please note, until the supporting documents are supplied, the enrolment application is not complete and we are unable to submit it for processing.

Applications can be submitted in the front office during the school term between 8:30am and 3:30pm or emailed to brookman.ps@education.wa.edu.au with the following Supporting Documentation.

PP – Year 6 Enrolment Procedure

An eligible child whose place of residence is within the intake area is guaranteed enrolment in the compulsory years of primary schooling (Pre Primary to Year 6). Children whose usual place of residence is not in the local intake area require an application for enrolment and are accommodated where possible based on capacity and facilities available.

PP – Year 6 Application Form

Applications can be submitted in the front office during the school term between 8:30am and 3:30pm or emailed to brookman.ps@education.wa.edu.au with the following Supporting Documentation.

    1. Proof of Address:

    One of the following items may be provided as proof of residence:

    • utilities payment advice
    • lease agreement – minimum lease of three months
    • proof of ownership of property (for example rates notice, deeds, mortgage)
    • driver’s licence
    • statutory declaration – Statutory Declaration Form

    2. Student documentation required:

    • Your child’s birth certificate 
    • Immunisation History Statement from your Medicare online account . Kindergarten children must be ‘up to date’ with all scheduled immunisations for their age to be able to enrol. For further information visit https://ww2.health.wa.gov.au/immunisationenrolment 
    • Family Court orders (if applicable)
    • Medical records where applicable (eg asthma, allergies)

    If your child is not born in Australia or both parents are born overseas, the following documents must be provided:

    • Passport
    • Evidence of date of entry into Australia
    • Current Visa documents – (Visa Grant Notice)
